Here’s video example of incomplete expand/collapse from the Pane Toolbar. This Pane is zoomed to the top level of the document but I think the filter stack causes problems.

If I “Reset” the filters, expand/collapse button functions correctly.
Note that when I add the “shift+click” item, it comes in as a Heading even though I clicked the “no prefix” new-item button AND the filters I’ve applied do not include Headings. I see that behavior a lot. Normally I get over it because it’s connected to nontrivial filter configurations – but in those cases I at least get unwanted prefixes that do appear in the filters. This example is more inexplicable than usual. Regardless of your decision on how to handle this behavior in the future (no change, fix it, or eliminate it) – when I click a button for a new item that explicitly indicates a prefix, I expect to get that prefix.
